1. Does Allendale offer an Adult Health Clinic?

The Board of Health provides a free health clinic for Allendale adult residents the fourth Thursday of every month except for November and December at the Borough Hall, 500 W. Crescent Avenue, Council Chambers.

Services include: *Health history evaluation *Blood pressure measurement *Height and weight measurement *Individualized counseling and education *Referral and follow-up *Confidential consultation with a Public Health Nurse.

2. I have a dead bird on my driveway, what do I do?

Use rubber gloves and/or a shovel and double bag it in plastic and dispose of it in your trash receptacle. If it is a Blue Jay or Crow notify the Allendale Board of Health.

3. There is a bat in my bedroom, what do I do?

Leave the room however try not to let the bat escape. Close off the room, keep all family members and pets out, and contact the Allendale Police Department for further instruction. Bats can transmit rabies and should it escape without testing can result in the necessity for rabies shots.

4. How do I handle an animal bite?

All animal bites, whether it is animal to person or animal to animal, MUST be reported to the Allendale Board of Health by telephoning directly or by notifying the Allendale Police Department as soon as possible.

5. What do I need to do when opening a new food establishment?

Contact the Allendale Board of Health to set up an inspection with our Sanitarian prior to opening. You will be given information on Food Handler's Courses and Food Manager's Courses. A food license is mandatory for operation.

6. Where do I apply for a marriage or civil union license?

You apply for a marriage license application in the town in which the bride resides. Licenses issued in New Jersey are only valid for use in New Jersey. Of you are a New Jersey resident planning an out-of-state ceremony, contact the Registrar in the town where the ceremony will take place.

7. Where do I obtain a certified copy of my Marriage Certificate or Civil Union Certificate?

Contact the Registrar in the town where the wedding ceremony took place (which is not necessarily the same place where you applied for your license). You will need to complete an application form and supply valid ID. A fee of anywhere between $10.00 and $15.00 is required for each certified copy.

8. Where do I obtain a certified copy of a Birth Certificate?

A certified copy of a Birth Certificate is obtained in the town in which the child was born. An application with exact information would be required with two forms of valid ID.

9. Where do I obtain a certified copy of a Death Certificate?

A certified copy of a Death Certificate is obtained in the town where the person dies, not where he lived. If he resided in Allendale but died in the hospital in Ridgewood, you would go to Ridgewood's registrar to obtain a copy of the Death Certificate. An application with exact information would be required with two forms of valid ID.

10. Who do I call to remove live wildlife from my property?

Certain wildlife is protected species. Therefore, you must contact either the Bergen County Animal Shelter or a licensed Wildlife Specialist.

11. Does Allendale offer a Well Baby Clinic?

A Child Health Clinic is located in Waldwick. The program is for Allendale residents and is for well children only whose parents do not have insurance coverage. A physician and a registered nurse are in attendance at each clinic. Children are given physical examination and any immunizations which are required provided there is no medical insurance coverage. Developmental and nutritional assessments are also given. Please contact our Board of Health at (201) 818-4400 ext. 211.

12. How do I safely dispose of my used syringes?

Needles and/or syringes must be placed in a protective container made of metal, tin or hard plastic such as a coffee can, detergent or Clorox bottle. The lid must be placed back on, taped and marked medical waste and/or sharps container. This can be disposed of safely in your regular trash pickup. Englewood Hospital and Holy Name Hospital offer a program whereby sharps containers are provided to the individual. When they are returned fill they are exchanged for an empty one.

13. What do I need to do for a special day event?

The Board of Health must also be notified of any temporary food events.

14. What do I do if I have a Board of Health complaint?

All food establishment complaints or public health nuisance complaints can be email to the Board of Health at manus@allendale.org or called into the Board of Health at (201) 818-4400 ext. 211.
